Before you start out on a journey, you should have a plan. Failing to plan is planning to fail and when it comes to planning your PATH to Wellness, you deserve a detailed plan that is tailored to your individual strengths and your specific needs. You also need to examine why other PATHs were not right for you and what core beliefs and underlying issues make this your best PATH. Therapists are the client-centered and solution-focused planners who will assist you in examining those issues and planning for your future success.

Your therapist will work with you to understand why you sought therapy. A specific issue may have led you to seek counseling. You may be aware that you are coping with a mental health or substance abuse issue and would like different strategies to use. Or you may have a strong baseline level of mental health but would like to invest in your well being from a maintenance or prevention perspective.

Your therapist will examine your personal history and current situation. The therapist will often ask you a series of questions about your life. It may be important to examine family situations and how they play an important role in who you are today. What is most important is that your therapist will apply these past situations to your current level of functioning.

Your therapist will discuss any current symptoms. Other than knowing the reason you sought therapy, it is important to find out if you're suffering in any way. A problem isn't a problem unless it impacts your regular functioning or is holding you back in some way. Once the issues have been identified, your therapist will work collaboratively with you to reduce or eliminate barriers to your success.

Case Management

If a Therapist is responsible for developing a plan for your journey, Case Managers are the guides that will accompany you on every step of your PATH to wellness. Case management is a professional and collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual's health needs. For the case manager, the highest level of responsibility is to the client. As positive advocates on behalf of these individuals, case managers help coordinate care that is safe, timely, effective, efficient, equitable, and most importantly, client centered.

The case management process includes the following nine steps:

  • Screening
  • Assessing
  • Stratifying risk
  • Planning
  • Implementation of your service plan/care coordination
  • Facilitating follow up
  • Transitional care
  • Communication before, during, and after all transitions
  • Ongoing evaluation

Throughout the case management process, case managers act with consideration for the individual's cultural beliefs, interests, wishes, needs, and values. Case managers are responsible for helping clients and their support systems evaluate and understand care options, determine what is best to meet their needs, and institute action to achieve their goals and meet their interests and expectations. In addition, the case management process assists health care organizations and professionals to meet their goals, such as delivery of safe, quality, cost-effective, and efficient care, as well as demonstrating adherence to regulatory and accreditation standards.
